Pre-enrollment Visit

To place your child on the waiting list, simply download this form and mail it in to: P.O. Box 24533, Chattanooga, TN, 37421 with your $50 non-refundable waiting list fee. We encourage you to schedule a tour at the same time or shortly after you join the waiting list; a pre-enrollment visit is required by state regulations. When you are called from the waiting list you will have 24 hours to respond with your decision.

enrollment & Admissions

The following are items required for Enrollment and Admissions

• Conduct a Pre-Enrollment Visit (instructions above)
• Complete an Application Packet
• Provide proof of a current physical exam for the child(ren)
• Provide an up to date immunization record for the child(ren)
• Provide a copy of the child’s Birth Certificate and Social Security Number.
• Sign Personal Safety Notification Form – ages 3 to 5 years
• Sign Parent Contract/Policy Agreements
• Sign Expulsion Policy
